Registering as a New Patient
We are currently open to accept new patients to our list.
If you decide you would like to register here at Bushloe End, we would ask that you call into the surgery and complete a registration form. Alternatively you can download the form from this page, print, complete and bring into the Surgery.
If you wish to register a child or children aged 0 - 16 you will also need to complete a separate child registration form, available to download below.
You will also need to provide some documentation with your present address, such as a current utility bill and your NHS number which you can get from your previous GP if you do not have this.
If you are on regular medication, it may be useful to hand over a list of your medication that you take.
When you join us we request your medical notes from your previous GP and summarise them onto our computer, however, this may take several weeks.
